Hearts at Home Senior Care recognizes that strokes are the leading cause of long-term disability and the leading PREVENTABLE cause of disability. Therefore, we’ve developed a Stroke Care Specialty Program designed to improve client outcomes, improve quality of life, and prevent hospital readmission. All caregivers are provided specialized training during the orientation process and ongoing education through our training partner. The Hearts at Home Stroke Care Specialty Program is directed by a licensed Physical Therapist with significant experience treating patients with stroke in a variety of settings.
Education, monitoring and professional home assistance will help improve patient outcomes, quality of life, and reduce readmission.
